Brockton Area Multi-Servi is seeking a skilled and compassionate Licensed Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to individuals with complex medical needs, developmental disabilities, acquired brain injury, and/or mental health issues in a community-based setting.
Key Responsibilities- Provide Skilled Nursing Care
- Administer medications and treatments following established protocols and regulations.
- Complete daily written notes, data tracking, and skilled assessments.
- Follow medical protocols, treatment plans, and medication treatment plans.
- Support Daily Living Activities
- Assist with daily living activities, such as bathing, dressing, and grooming.
- Provide physical support, assistance with lifting and transferring individuals as needed.
- Collaborate with the Care Team
- Work collaboratively with the direct care and supervisory team to ensure the health status of individuals.
- Provide training to staff and individuals on relevant areas, such as health, hygiene, safety, and diet.
- Ensure a Safe and Healthy Environment
- Ensure safety standards and infection control protocols are followed.
- Perform other related duties as needed or assigned by supervisor or designee.
- Possession of a Massachusetts license as a nurse.
- 2 years of experience working with individuals with acquired brain injury or similar disabilities preferred.
- Basic computer literacy, including the ability to record data and use electronic time and attendance software.
