Job Summary
The Day Program Case Manager plays a vital role in our Day Program Services, building relationships with individuals and their support teams to ensure seamless coordination of services.
Key Responsibilities
- Collaborate with departments, individuals, families, and funding sources to ensure services meet individual needs and promote choice.
- Conduct assessments and evaluations to determine support levels and develop support plans.
- Monitor follow-through and effectiveness of support plans, providing guidance to department staff and interdisciplinary teams.
- Participate in planning meetings, advocating for individuals' needs and contributing to overall department goals.
- Provide personal care and assistance to individuals, consistent with their needs, and provide coverage for groups as needed.
Requirements
- High school diploma required; BA degree in Human Services or related field preferred.
- At least 1 year of experience supporting adults with developmental disabilities is preferred.
- A passion and dedication to supporting individuals with developmental disabilities is essential.
- Valid driver's license, reliable transportation, and acceptable driving record.
- Must pass company background and reference checks.
