Job Summary

This position is responsible for collecting and verifying the current home medication list of patients admitted to Blessing Hospital. The successful candidate will gather this information from various sources, including patient interviews, home medication lists, and retail pharmacies.

Key Responsibilities
  • Collect and verify patient medication lists
  • Interview patients and/or significant others to gather medication information
  • Document medication lists and work with pharmacists to complete initial reviews
  • Collaborate with healthcare teams to ensure accurate and up-to-date medication information
Requirements
  • High School Diploma or equivalent
  • Completion of Pharmacy Technician Training Course
  • Computer skills and excellent written and verbal communication skills
  • Illinois registration as a Pharmacy Technician and certification by the National Pharmacy Technician Certification Board or equivalent
