The Physical Therapist collaborates with an interdisciplinary team, providing valuable insights and support in the realm of Physical Therapy.


Key Responsibilities:

  • Analyze resident information to assess its influence on treatment strategies.
  • Conduct comprehensive functional assessments using standardized methodologies, including evaluations of home and vocational needs.
  • Create and execute tailored treatment plans that address individual resident requirements and establish attainable functional objectives.
  • Adhere to documentation and treatment protocols, ensuring timely and precise record-keeping.
  • Guide and supervise assistants in treatment methodologies while they engage with residents.
  • Document progress in resident records after each therapy session.
  • Adjust treatment plans as necessary throughout the therapeutic process.
  • Prepare clear and concise discharge summaries that outline treatment progress.
  • Introduce residents to available program services and treatment options, ensuring they understand any associated risks.
  • Collaborate with nursing staff, residents, and families to optimize the use of residents' functional capabilities.
  • Educate residents and their families on home exercise regimens and therapeutic practices to continue post-treatment.
  • Assist in referrals for additional services as needed after discharge, maintaining knowledge of community resources.
  • Participate in rehabilitation team meetings to discuss resident progress and needs.
  • Engage in departmental meetings and training sessions as required.
  • Pursue continuing education opportunities to maintain clinical expertise.
  • Contribute to quality improvement initiatives.
  • Promote the services of Heritage Healthcare through community engagement.
  • Comply with all regulatory and professional standards set by Heritage Healthcare.
  • Provide feedback to the Rehab Manager regarding the performance of assistants as appropriate.
  • Follow facility policies and engage in quality improvement and safety initiatives.
  • Perform additional duties as necessary to ensure high-quality resident care.
  • Understand reimbursement processes and manage therapy resources effectively to meet resident goals.
  • Demonstrate knowledge of relevant regulations and compliance standards.
  • Embody the commitment to Compassionate Care, presenting a professional image that instills confidence in our Rehab Program.
  • Work independently while recognizing when direct supervision is necessary, and assist colleagues as needed.
  • Maintain expected productivity levels as discussed for your role.

Qualifications:

  • Bachelor's or Master's degree in Physical Therapy.
  • Current licensure in the relevant state.
  • Preferred experience in a similar setting serving comparable resident populations.
  • Certification in specialized areas, if applicable, with demonstrated competency.
  • Strong communication and interpersonal skills, both verbal and written.
  • Proficient in applying physical therapy principles, methods, and equipment.
  • Ability to assess resident needs and formulate comprehensive care plans.
  • Familiarity with accreditation standards and compliance requirements.
Physical requirements include:

Standing:
Ability to maintain an upright position for extended periods.

Pushing:
Ability to exert force against objects, moving them as necessary, with a force of up to 35 lbs.

Lifting:
Capability to lift objects up to 50 lbs independently, and over 50 lbs with assistance.

Stooping:
Bending forward and down for extended periods.

Reaching:
Extending arms in various directions.

Balancing:
Maintaining equilibrium on various surfaces.
