About the Role

Fairview Health Services is seeking a highly skilled Licensed Mental Health Professional to join our Crisis Response Team as a Crisis Intervention Specialist. In this role, you will provide critical support to individuals in crisis, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Clinical Services:
    • Initiate client-centered consultation with internal and external providers to coordinate care
    • Provide crisis call support and determine appropriate level of care for clients
    • Complete comprehensive behavioral assessments to identify patient needs and recommend a level of care
    • Provide psychotherapy for clients
    • Assess and complete the DEC (Diagnostic Evaluation Center) assessment form for all crisis patients
    • Determine appropriate disposition, coordinate care, and provide discharge summaries for patients
    • Collaborate with patients, medical doctors, and families to create treatment plans based on medical necessity and patient preference
    • Provide primary communication to collaborating partners, such as hospital emergency departments, schools, primary care clinics, and other treatment providers
    • Adhere to all policies and procedures
    • Maintain knowledge of available community resources and procedures for hospitalizations
  • Record Keeping:
    • Obtain necessary documents, information, and signatures from patients to complete assessments and coordinate care
    • Conduct risk assessments, de-escalation techniques, and mental status evaluations
    • Promptly and accurately document patient information, major crisis or treatment issues, and expected outcomes based on medical record-keeping standards
  • Service to Internal and External Customers:
    • Respond to queries pertaining to services that can be provided to best meet patient needs
    • Troubleshoot technical issues with telehealth sites
    • Locate and schedule routine and crisis appointments for patients within established timeframes
    • Establish and maintain positive and professional working relationships with internal staff, external service providers, and other state and county agencies
    • Engage in appropriate professional development through continuing education, membership in professional organizations, and licensure as eligible
Requirements
  • Education: Master's Degree from an accredited school in Social Work, Psychology, or a closely related discipline
  • Certifications: Basic Life Support Certification
  • Licensure: Valid Minnesota driver's license, access to a car during designated hours, proof of auto insurance coverage
  • Professional Licenses: Licensed in one of the following in the State of Minnesota: Licensed Psychologist (Doctoral or Master's), Licensed Independent Clinical Social Worker, Licensed Marriage Family Therapist (Doctoral or Master's), or Licensed Professional Clinical Counselor (Doctoral or Master's)
  • Skills: Good working knowledge of electronic health records and comfort level with utilizing computer-based platforms for assessments, as well as proficiency with computers
  • Experience: Three or more years' experience in crisis counseling/situations or clinical therapy
