Job Summary

We are seeking a highly skilled and experienced RN Admissions Nurse to join our team at Robinson Terrace Rehabilitation and Nursing Center. As a key member of our healthcare team, you will be responsible for coordinating the admission process, assessing patient needs, and developing individualized care plans.

Key Responsibilities
  • Initiate admission assessments on new patients and hospital returns, and develop initial care plans.
  • Assist in the development and direction of the Nursing Department as directed by the Director of Nursing.
  • Assess patient needs and coordinate all components of the admission process with the interdisciplinary team.
  • Responsible for auditing records to ensure compliance with facility standards.
  • Participate in discharge planning as necessary.
  • Review and evaluate patient care plans as conditions warrant a change.
  • Perform required charting according to policy and procedure.
  • Complete Minimum Data Set (MDS), care plans, and other documents as assigned.
  • Maintain the confidentiality of all patient information as per HIPAA and corporate guidelines.
  • May be assigned to supervise, direct, and/or provide patient care as necessary.
Requirements
  • Work day will be flexible 8-hour shifts with core hours being 1pm-6pm.
  • Must have an active and unrestricted New York State Registered Nurse License.
Preferred Qualifications
  • Excellent clinical assessment skills.
  • Ability to make independent decisions.
  • Strong leadership and supervisory skills.
  • Computer savvy; proficient in MS Office (Excel, Word, Outlook).
  • Long-Term Care Experience Preferred.
  • Excellent communication and interpersonal skills.
