Rehabilitation Case Manager

MercyOne Clive Rehabilitation Hospital is seeking a skilled Rehabilitation Case Manager to join our interdisciplinary team. As a key member of our team, you will play a vital role in delivering exceptional patient care and outcomes.
Key Responsibilities:- Collaborate with healthcare professionals to develop and implement individualized treatment plans
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
- Coordinate and facilitate patient care team conferences to drive treatment plan development and refinement
- Monitor and report on patient progress, identifying areas for improvement and opportunities for growth
- Current licensure as a Registered Nurse, Social Worker, or other healthcare professional
- Minimum 2 years of experience in inpatient case management or rehabilitation settings
- Strong communication and interpersonal skills, with ability to work effectively in a team environment
- Basic computer skills and proficiency in electronic medical records
