Rehabilitation Case Manager

MercyOne Clive Rehabilitation Hospital is seeking a skilled Case Manager to join our interdisciplinary team. As a Case Manager, you will play a vital role in delivering high-quality patient care and achieving exceptional outcomes.
Key Responsibilities- Collaborate with the healthcare team to develop and implement individualized care plans
- Conduct thorough assessments and identify potential barriers to care and discharge
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
- Coordinate weekly patient care team conferences to refine treatment plans and achieve identified patient goals
- Review patient records to identify potential missed comorbid conditions and communicate findings to the HIM team
- Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist)
- Certification in Case Management or Rehabilitation Nursing preferred
- Minimum of 2 years experience in an inpatient setting, preferably in acute/rehabilitation hospital
- Effective oral and written communication skills in English, additional languages a plus
