Key Responsibilities:- Obtain comprehensive and accurate patient home medication lists as part of the admissions process.
- Coordinate with the Clinical Pharmacy Manager and ED Pharmacists to ensure timely and accurate medication reconciliation.
- Assist with deliveries of code trays to the Emergency Department and participate in ongoing quality improvement programs.
- Communicate effectively with physicians, nursing staff, and other clinicians to ensure seamless medication reconciliation.
- Perform medication history assessments, verify patient allergy information, and document medication lists and allergy information in the electronic medical record.
- High School diploma required.
- Minimum 2 years of Pharmacy Technician experience.
- Familiarity with medical and drug terminology and abbreviations.
- Excellent communication and computer skills.
- Must be CPT or PTCB certified or a registered pharmacy intern with the state of Massachusetts.
- Must be registered with the Massachusetts Board of Pharmacy as a Pharmacy Technician or Intern.
