About the Role

We are seeking a highly skilled and compassionate Intensive Care Coordinator to join our team at AltaPointe Health Systems.

Responsibilities

  • Respond promptly to crisis calls and coordinate the Child/Adolescent Crisis Team's response to crises.
  • Demonstrate the ability to recognize crisis states and deescalate or resolve situations effectively.
  • Communicate effectively with Access to Care and provide crisis response when needed.
  • Assess consumer needs using the CANS and identify goals and plans for appropriate services.
  • Provide linkage to community resources and monitor consumers to secure placement in the least restrictive environment.
  • Evaluate consumers for discharge planning and track clinical outcomes according to contractual agreement.
  • Collaborate with treatment team members and outside sources to deliver case management services.

Supervision and Consultation

  • Seek clinical supervision and consultation as needed and accept suggestions for improvement.
  • Enhance case management skills and document in a timely fashion per AltaPointe policy.

Clinical Record Keeping

  • Document CANS assessments, individualized service plans, and case disposition.
  • Document legibly and maintain accurate records.

Case Load Management

  • Effectively manage caseload based on consumer needs and meet AltaPointe productivity standards.

Qualifications

Bachelor's degree in a mental health discipline and valid driver's license required. Knowledge of child/adolescent specialty and CANS assessment required within six months of employment.
