Position Overview:
We are seeking dedicated Registered Nurses to join our team as Care Coordinators. This role involves conducting thorough assessments for elderly clients, ensuring all necessary documentation for reassessments is completed accurately. Each assessment, whether conducted via telephone or in person, may take up to two hours.

Key Responsibilities:
  • Perform nursing assessments for elderly clients.
  • Complete all required documentation for reassessments.
  • Engage in both telephonic and face-to-face assessments.

Work Schedule:
Flexible shifts are available, with hours ranging from 6 to 16 hours per week, based on your availability from Monday to Friday. This position offers part-time hours suitable for supplemental income.

Qualifications:
  • Current California RN Licensure is mandatory.
  • Experience with the adult and older adult population is preferred but not essential.
  • Strong organizational and writing skills are crucial.
