Job Summary

Home & Health Care Management is seeking a skilled Registered Nurse to join our interdisciplinary care management team. As a Registered Nurse, you will work with clients who are receiving home and community-based services, providing comprehensive care and support to ensure the best possible outcomes.

Responsibilities
  • Review medical records and develop care plans to ensure the right amount and type of care meets the client's medical needs.
  • Communicate with healthcare providers, clients, and caregivers to coordinate care and services.
  • Work with an interdisciplinary team to provide comprehensive care and support to clients.
  • Institute tracking of quality indicators and reports to improve patient outcomes.
  • Coordinate with Case Management team members to ensure seamless care and support.
Requirements
  • Current California State license as a Registered Nurse.
  • Valid California Driver's License.
  • Eligible to be insured under our liability policy with a clean DMV report.
  • Proficiency using software programs, and familiar with Microsoft office product including Word and Excel.
  • Ability to work independently and with others at all levels within the organization, and collaborate effectively.
Preferred Skills
  • Professional behavior when on duty.
  • Discharge planning, home health or hospice experience.
  • At least one year of work experience as an RN.
Service Region/Location

This role is located in Redding, serving Shasta and surrounding counties.

Schedule

This role is full-time, 40 hours per week, from 8am to 5pm Monday through Friday with an hour for lunch from 12pm-1pm.

Travel

This role involves some travel to and from clients' homes.

Physical Requirements

Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity.

Compensation

Pay range is $43 to $53 per hour. Exact compensation may vary based on skills and experience.

Benefits

You will be reimbursed for the mileage you travel from the office to participant's homes.

You will receive a monthly phone stipend.

On the first day of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).

Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
