- Collaborate with prescribers, therapists, and case managers to develop and implement individualized care plans.
- Administer IV therapy, as per UPMC's LPN Practice Guidelines and state regulations.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Identify opportunities for process improvement and contribute to the development of innovative solutions to enhance patient care and satisfaction.
- Build strong relationships with patients, families, and colleagues, promoting a culture of empathy and respect.
- Active LPN licensure in good standing, with the ability to maintain licensure in the state where the facility is located.
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Comprehensive Crisis Management (CCMC) certification.
- Act 31 Child Abuse Reporting with renewal.
- Act 33 with renewal.
- Act 34 with renewal.
- Act 73 FBI Clearance with renewal.
- Prior experience in a mental health setting.
