Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Community Health Net. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and promoting the well-being of our clients.

Key Responsibilities
  • Conduct thorough health histories and assessments to determine client medical conditions.
  • Collaborate with medical providers to develop and implement effective care plans.
  • Coordinate referrals to other health and social agencies to ensure seamless patient care.
  • Provide health education and support to clients and their families.
  • Maintain accurate and up-to-date medical records and documentation.
  • Assist with administrative tasks, such as registration, discharge, and scheduling.
  • Float to other Community Health Net sites as needed to cover nursing duties.
Requirements
  • Current and valid Pennsylvania LPN license and CPR certification.
  • Ability to use computers and Windows-based software to enter data and develop reports.
  • Strong organizational and communication skills.
  • Knowledge of medical terminology and healthcare practices.
