The APP conducts comprehensive physical assessments, engages with patients through various communication methods, and collaborates with the healthcare team to ensure seamless patient care aligned with established quality standards. A commitment to delivering exceptional service to the Vein & Vascular Specialists is essential.
KEY RESPONSIBILITIES:
- Conduct thorough assessments of patients, including interviews, physical examinations, and the ordering of diagnostic tests.
- Provide clear instructions and guidance regarding medical care to patients.
- Identify and report any urgent medical issues requiring immediate attention.
- Maintain and update patient diagnoses and collaborate with physicians to develop or manage care plans.
- Document clinical findings, diagnostics, and care plans accurately and promptly in the electronic health record (EHR).
- Manage incoming calls for physicians, addressing inquiries in accordance with patient care plans.
- Coordinate referrals and consultations with other healthcare professionals as necessary.
- Communicate follow-up information effectively to referring physicians.
- Educate patients and facilitate communication among healthcare providers, caregivers, and staff.
- Act as a liaison between patients and the medical staff.
- Perform routine diagnostic and therapeutic procedures as required.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Deliver compassionate care to patients and their families, ensuring confidentiality and adherence to safety protocols.
- Participate in departmental meetings and work collaboratively with colleagues to achieve goals.
- Stay updated on professional knowledge through literature and educational programs, applying evidence-based practices relevant to vascular patients.
- Assist surgeons during endovascular or sclerotherapy procedures as needed.
- Engage in the development of new programs for the Vein and Vascular center.
- Participate in performance improvement initiatives and emergency preparedness activities.
- Maintain a clean and organized work environment, adhering to safety principles.
- Demonstrate proficiency in using technology for documentation and communication.
- Maintain required certifications and participate in ongoing professional development.
- Provide training and support to new staff as needed.
- Ensure compliance with billing and coding guidelines for patient encounters.
QUALIFICATIONS:
Education: Graduate of an accredited Physician Assistant or Nurse Practitioner Program.
Experience: A minimum of three years as a Physician Assistant or Nurse Practitioner, with at least one year of experience in vascular care preferred. Strong new graduates interested in vascular surgery will be considered.
Licensure/Certification: Must be licensed in the state of NH as a Physician Assistant or Nurse Practitioner. BLS and ACLS certifications are required, maintained through the American Heart Association.
