Job Summary

Sharp HealthCare is seeking a skilled Healthcare Partner - Clinical Support Specialist to join our team. As a key member of our healthcare team, you will provide exceptional patient care and support to our patients and families.

Key Responsibilities
  • Clinical Competency
    • Under the supervision of a licensed nurse, perform at least one higher-level competency from the following:
    • EKG
    • Lift Team/Mobilization
    • Integrative Therapy
    • OB Tech
    • Mother/baby dyad care
    • Orthopedic Tech
    • Other specialized skills such as surgical support/arterial lines/sterilization processes
  • Teamwork and Communication
    • Greet and welcome patients and families by telephone and in-person.
    • Obtain information from visitors/callers, direct as appropriate or take complete and accurate messages.
    • Act on requests for customer assistance.
    • Use scripts as appropriate, including answering phones, transferring calls, and service recovery.
    • Answer call lights within four (4) rings or call into patient rooms to validate requests.
    • Answer phone within four (4) rings and transfer calls as indicated.
    • Respond to all inquiries with a timeliness that promotes customer satisfaction.
    • Coordinate with physicians and department team members via paging system or phones.
    • Assist in directing others during crisis intervention (codes, disasters, etc.).
    • Communicate effectively with all levels of staff and use the chain of command per unit guidelines.
    • Prioritize workload in accordance with patient needs and staffing patterns, as appropriate.
    • Validate understanding of new patient needs or changes in assignment.
    • Communicate with other staff to develop a plan for the best use of resources.
    • Ask for assistance to get workload completed.
    • Report to the licensed nurse or supervisor when unable to respond to the number of current demands.
    • Seek feedback from coworkers about strategies to complete work more efficiently.
    • Complete and sign appropriate documents accurately and timely.
  • Admissions, Discharge, and Transfer Responsibilities
    • Ensure complete room setup for new admissions (bedpan, urine container, tissue paper, basin, fresh water, etc.).
    • Meet and greet new patients and families on admission to the unit.
    • Orient patients and families to the facility/room.
    • Inventorize and document patient personal belongings on an inventory sheet upon admission and update as needed.
    • Account for patient belongings during transfer and discharge processes.
    • Transfer/transport patients to activities, therapies, and other departments on time and in a safe manner.
    • Remove all patient equipment and supplies upon discharge.
    • Discharge patients from IDX within the expected timeframe of patients leaving the unit.
    • As directed by the RN, appropriately enter orders related to equipment, supplies, and dietary needs; seek RN clarification when needed to ensure accuracy.
  • Patient Care Activities
    • Complete and sign appropriate documents accurately and timely.
    • Provide patient care within the scope of responsibilities.
    • Provide and document daily care of patients per unit standards.
    • In monitored units, follow telemetry process and collaborate with central telemetry for initiating/discontinuing monitoring.
    • Communicate or question requests outside of scope of practice to licensed nurses.
    • Identify and communicate new resident/patient care issues/concerns.
    • Communicate changes in patients/residents condition.
    • Take rapid action in life-threatening situations and immediately notify the licensed nurse.
    • Communicate plans to transfer patients/residents to other departments or discharge to home.
  • Safety
    • Comply with universal precautions and hospital infection control policies.
    • Use proper body mechanics and safe patient mobilization equipment to ensure patient safety and avoid personal injury.
    • Follow safety procedures required for equipment use.
    • Report observed hazards and unsafe practices.
    • Report personal injury within two hours after the incident happened, and complete the ART and state claim forms.
    • Identify and communicate patients who are at risk for injury to self and others.
    • Comply with hospital policy in management of patients in restraints.
    • Label/store, ensure safety of patient belongings (hearing aids, glasses, dentures).
    • Participate in auditing and safety monitoring programs per unit standards.
  • Medical Record/Information Management
    • Obtain chart inserts for admissions and maintain charts, ensuring additional forms are available and placed in charts.
    • Obtain previous charts from medical records.
    • Dismantle charts to return to Medical Records, including old charts.
    • Request medical records, films, or other medical information from other facilities as needed.
    • File interdisciplinary and diagnostic printouts in chart per standards.
    • Date, time, and initial all entries.
    • Assist team members in locating patient data information.
    • Collect and prepare patient data necessary to support inter-facility transfers.
    • Accurately update electronic information systems (i.e., tracking shell, Stafflink, NaviCare Hill-Rom systems).
    • Ensure timely teletracking updates/requests.
    • Enter accurate disposition of patients requiring admission, transfer, or discharge per unit time guidelines.
    • Coordinate scheduling of tests and retrieving results to support patient progress.
    • Retrieve diagnostic study, lab results, and other relevant patient information supportive of patient assessment.
    • Prepare/stock downtime packets.
  • Equipment and Supplies
    • Obtain, discontinue, store, and clean equipment according to policy/procedure and manufacturer guidelines.
    • Enter work order to facilitate repair of equipment, report urgent or unresolved equipment malfunction to lead/manager or the appropriate department (Biomed, Supply Chain Services, etc.).
    • Manage and operate equipment safely.
    • Ensure integrity/sterility of supplies.
    • Check and restock bedside units/exam rooms after each patient and prn.
    • Restock supplies (e.g., clerical, patient care, and dietary).
    • Track equipment availability: logs, assigns, and tracks equipment.
    • Ensure compliance with safety regulations (e.g., equipment does not block fire exits).
    • Follow durable, re-usable medical equipment protocols.
Requirements
  • Less than 1 year of related clinical experience.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association.
Preferred Qualifications
  • H.S. Diploma or Equivalent.
  • Other Comparable certified nursing assistant course.
  • 1 year of experience as a unit clerk/secretary in an acute care setting.
