**Job Summary**

We are seeking a highly skilled and compassionate Shared Living Coordinator to join our team at Advocates. As a Shared Living Coordinator, you will play a critical role in supporting individuals with disabilities to live independently in their own homes.

**Key Responsibilities**

* Provide support and guidance to Shared Living Providers to ensure individuals receive necessary resources and services.

* Manage a caseload of individuals living in Shared Living settings, ensuring their needs are met and their rights are protected.

* Coordinate services with DDS Service Coordinators, families, and guardians to ensure seamless support.

* Develop and implement Individual Service Plans (ISPs) that promote individuals' health, well-being, and independence.

* Monitor and oversee the implementation of ISPs, ensuring timely documentation and compliance with regulations.

* Collaborate with the Shared Living Director to ensure regulatory compliance and maintain accurate records.

**Requirements**

* Bachelor's degree in a related field or significant experience in the DS/MH area.

* Three years of experience in training and system monitoring.

* Valid driver's license and reliable vehicle.

* Additional specific experience may be required.
