Job Summary:The LPN will be responsible for delivering direct nursing care to residents, supervising nursing assistants, and collaborating with the healthcare team to ensure seamless care delivery. You will work closely with our interdisciplinary team to develop and implement individualized care plans, addressing the unique needs and goals of each resident.
Responsibilities:- Deliver high-quality nursing care to residents, including administering medications, performing treatments, and providing emotional support.
- Supervise and delegate tasks to nursing assistants, ensuring that care is delivered in accordance with established policies and procedures.
- Collaborate with the healthcare team to develop and implement individualized care plans, addressing the unique needs and goals of each resident.
- Participate in resident assessments, care planning, and discharge planning to ensure continuity of care.
- Communicate effectively with residents, families, and healthcare team members to ensure that care is delivered in a compassionate and respectful manner.
- Document resident care and progress in accordance with established policies and procedures.
- Current LPN licensure in the state of practice.
- Excellent technical, assessment, and documentation skills.
- Good leadership qualities, with the ability to supervise and delegate tasks effectively.
- Organized and detailed in work performance, with the ability to prioritize tasks and manage time effectively.
- Good communication skills, with the ability to communicate effectively with residents, families, and healthcare team members.
- Genuine caring for and interest in elderly and disabled people in a nursing facility.
