Job Summary:
We're seeking an experienced Registered Nurse to join our team as a Nurse Care Manager. In this role, you'll be responsible for assessing, diagnosing, planning, implementing, and evaluating patient care through outcome identification and evidence-based practice.
You'll coordinate patient care and communicate extensively with the healthcare team, patients, and their families. You'll maintain high standards of professional ethics and participate in clinical decision-making. Your duties will include performing assessments, administering therapeutic interventions, managing rapidly changing situations, and educating patients and families about disease processes and individual care needs.
To succeed in this role, you must have valid NJ RN License, knowledge of nursing skills, hospital practices, procedures, and standards, and strong communication skills. You should be able to coordinate efforts of a team of care providers.
Responsibilities:
- Perform comprehensive patient assessments, leading to development, implementation, and evaluation of individualized care plans.
- Administer, monitor, and document therapeutic interventions and regimes.
- Effectively manage complex patient situations, prioritizing tasks and communicating with the healthcare team.
- Participate in clinical decision-making, collaborating with physicians and other healthcare professionals.
- Educate patients and families about disease processes, individual care needs, and wellness strategies.
- Delegate and monitor care rendered by other members of the nursing department and healthcare team.
Requirements:
- Valid NJ RN License
- Demonstrated knowledge of nursing skills, hospital practices, procedures, and standards
- Strong communication and teamwork skills
Estimated Salary: $85,000 - $110,000 per year, depending on experience
