Job Summary

Hillcrest Extended Care Services is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will play a vital role in providing top-quality care to our residents.

Key Responsibilities:

  • Provide basic nursing care, including vital sign checks, wound care, and medication administration.
  • Document patient care, including nursing interventions, patient responses, and progress towards goals.
  • Communicate effectively with RNs, physicians, and other healthcare team members to ensure seamless care.
  • Collaborate with other nurses and healthcare professionals to ensure residents' well-being.
  • Teach patients and their families about health information and provide written materials as needed.
  • Perform direct patient care, using established procedures, policies, and standards.
  • Administer medications according to policy and procedure.
  • Observe and document patients' responses to medications.
  • Respond to emergency situations in a prompt, precise, and professional manner.
  • Admit, transfer, and discharge residents as required.
  • Complete medical forms, reports, evaluations, and charting as necessary.
  • Receive phone orders from physicians and record on the Physician's Order Form.
  • Chart all reports of accidents/incidents involving residents.
  • Verify resident identity before administering medications.
  • Ensure adequate medications, supplies, and equipment are available.
  • Report needs to the Director of Nursing.
  • Notify the Director of Nursing of all drug and narcotic discrepancies.
  • Provide thorough, concise reports at change of shift.
  • Communicate pertinent information to the appropriate person in a timely manner.

Requirements:

  • Current LPN licensure in the state of employment.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Basic computer skills and familiarity with electronic medical records.
