Key Responsibilities:
  • Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, and physical exams.
  • Perform evidence-based practice screenings and point-of-care testing as needed.
  • Identify diagnoses and develop care management plans to address patient needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Travel up to 85% to visit patients in their homes.
Requirements:
  • Active NP or PA license and national certification in a relevant specialty.
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions.
  • Reliable transportation for home visits.
Preferred Qualifications:
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel up to 5% within licensed states for additional incentives.
