We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities:- Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, and physical exams.
- Perform evidence-based practice screenings and point-of-care testing as needed.
- Identify diagnoses and develop care management plans to address patient needs.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Travel up to 85% to visit patients in their homes.
- Active NP or PA license and national certification in a relevant specialty.
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions.
- Reliable transportation for home visits.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel up to 5% within licensed states for additional incentives.
