Key Responsibilities:
The Home Health Nurse, RN will be responsible for delivering skilled nursing care to patients who require intermittent professional nursing services.

Essential Functions:
1. Adhere to and promote 3HC's Compliance Program and Code of Ethics.
2. Foster positive interactions with both internal and external clients, ensuring a high level of courtesy, tact, and knowledge of agency services in all communications.
3. Manage assigned patient cases and collaborate with office staff to achieve favorable patient outcomes while adhering to strategic plans and budgetary constraints.
4. Conduct comprehensive patient assessments to determine appropriate care needs.
5. Maintain accurate electronic medical records and ensure timely updates.
6. Complete OASIS documentation at designated intervals, such as admissions and recertifications, while supervising Licensed Practical Nurses and paraprofessionals in accordance with regulatory guidelines.
7. Maintain consistent communication with supervisors regarding patient issues, following the established chain of command.
8. Ensure accurate billing for services rendered.
9. Demonstrate resourcefulness in utilizing agency resources effectively, monitoring waste, and ensuring the safe use of supplies and equipment.

Qualifications:
- Current Registered Nurse license to practice in North Carolina.
- Preferred: Minimum of one year of experience in an acute care setting.
- Preferred: At least one year of experience in Home Health.
- Valid North Carolina Driver's License.
