Clinical Liaison for Skilled Nursing Facility Partnerships

We are seeking a skilled Clinical Hospital Liaison to join our team at Aventura Health Group. The ideal candidate will be responsible for creating and maintaining successful relationships with hospital case managers, discharge planners, social workers, and physicians.
Key Responsibilities:
- Manage and coordinate the inquiry, referral, and admission process.
- Assist the team and Administrator in preparing and implementing marketing goals and objectives.
- Act as liaison between company and outside community agencies to generate a positive image and encourage referral activities.
- Informed of Medicare, Medicaid, and other insurance guidelines as pertains to position.
Requirements:
- Experience in nursing home admissions.
- Demonstrated sales experience in senior living, hotel/hospitality, or related healthcare environment preferred.
