As a key member of the Health Care and Rehabilitation Services (HCRS) team, the Care Coordinator - Behavioral Health plays a vital role in providing care coordination, case management, and resource connection services to individuals and families.
Key Responsibilities:- Coordinates internal and external referrals to ensure seamless care transitions.
- Utilizes the SBIRT model to identify and address mental health and substance use concerns.
- Assists individuals and families in identifying and prioritizing goals, developing time-limited care coordination plans, and measuring success.
- Provides care coordination and resource connection services, establishing relationships with community teams and providers.
- Maintains accurate and timely clinical documentation in the Electronic Medical Record.
- BA in Social Work or related field (or equivalent experience).
- Excellent communication and interpersonal skills.
- Ability to work well with a team and demonstrate flexibility in a fast-paced environment.
- Intermediate to advanced computer skills.
