We are seeking a compassionate and experienced Physical Therapist to join our Home Health program at Jefferson Healthcare. As a Physical Therapist, you will play a vital role in helping patients achieve their goals through exercises such as strength and flexibility training, muscle re-education, balance and gait training, pain relief, and the use of orthotic/prosthetic devices.
Key Responsibilities- Provide physical therapy services to patients in their homes, focusing on enhancing their well-being and reducing the need for hospitalization.
- Develop and implement personalized treatment plans to address patients' specific needs and goals.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement.
- Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to treatment plans.

- Jefferson Healthcare qualifies for Public Service Loan Forgiveness.
- Current Physical Therapist, WA State license in good standing.
- AHA BLS/CPR certification, active prior to start.
- Graduate of an approved PT program with a BS PT, MPT, MS PT or DPT.
- Valid Washington State driver's license and proof of auto insurance.
- Two years' experience in an outpatient setting, and/or one year inpatient or SNF, preferred.
