Key Responsibilities:
  • Monitor and assess the delivery of home health field services to ensure compliance with VNS Health guidelines and regulations.
  • Supervise the activities of Home Health Aides, providing training and support as needed.
  • Collaborate with the primary care physician and clinical team to perform patient assessments and develop care plans.
  • Identify continued training needs of Home Health Aides and document findings electronically.
  • Deliver start of care packets and provide patient reviews on initial visits.
  • Participate in quality assurance and education programs as requested by management.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in the State of New York.
  • CPR/BLS Certification required.
  • Associate's Degree in Nursing from an approved program required.
  • Minimum of two years in a clinical background as a Registered Nurse required.
