Nurse - Laredo

The Registered Nurse is responsible for planning, organizing, and directing hospice care. This role requires a strong emphasis on community health education and experience. The nurse builds on the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.
Key Responsibilities
Patient Care
- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Provide physical assessments and histories of current and previous illnesses.
- Deliver professional nursing care by utilizing the nursing process.
- Assess and evaluate patient status by writing and initiating care plans, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
- Use health assessment data to determine nursing diagnoses and develop care plans that establish goals based on nursing diagnoses.
- Initiate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed, and counsel patients and families in meeting nursing and related needs.
- Provide health care instructions to patients as appropriate and assist patients with activities of daily living.
- Act as a case manager and coordinate patient care for assigned caseloads.
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient conditions and care given.
- Record pain/symptom management changes/outcomes as appropriate and communicate with physicians regarding patient needs.
- Teach patients and families/caregivers self-care techniques and provide medication, diet, and other instructions as ordered by physicians.
- Work in concert with the interdisciplinary group and provide a safe environment for patients.
- Assist patients and families/caregivers and other team members in providing continuity of care.
- Attend interdisciplinary group meetings.
- Participate in on-call duties as defined by the on-call policy.
- Ability to work weekends.
- Ensure arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Fulfill the obligation of requested and/or accepted case assignments.
- Actively participate in quality assessment performance improvement teams and activities.
- Graduate of a National League for Nursing accredited school of nursing.
- Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing.
- Registered nurse with current licensure to practice professional nursing in the state.
- Demonstrates excellent observation, problem-solving, verbal, and written communication skills.
- Shows ability to organize and prioritize workload independently.
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Self-directed and able to work with minimal supervision.
