The Clinical Nurse Coordinator (CNC) plays a vital role in delivering high-quality, patient-centered care and coordinating all functions in the unit. In partnership with the management team, the CNC monitors and evaluates nursing care in accordance with established policies.
The CNC serves as a resource for staff and supports an unparalleled patient experience. With a focus on comprehensive, advanced care and treatment, the Stroke/Brain unit helps patients recover their health and functioning, and make the most of their capabilities.
About the UnitThe Stroke/Brain unit is a 16-bed unit focused on providing comprehensive care to patients with stroke and brain-related conditions. The unit offers a collaborative and supportive environment, with a strong emphasis on teamwork and patient satisfaction.
The unit is equipped with state-of-the-art technology and features a multidisciplinary team approach to care, including neurology, therapies (PT/OT/ST), case management, rehab, and nursing. The CNC will work closely with this team to coordinate care and develop individualized plans for each patient.
Responsibilities- Partner with a multidisciplinary team to coordinate, evaluate, customize, and deliver high-quality, patient-centered care.
- Assist with admission and discharge processes.
- Partner with subject matter experts and leadership team to support improved outcomes and service.
- Provide coaching, feedback, and training to improve performance and engagement.
- Assist with staff scheduling.
- Advanced Cardiac Life Support (ACLS) certification must be obtained within 30 days of employment start date.
- Basic Cardiac Life Support (BLS) certification must be obtained within 30 days of employment start date.
- Registered Nurse (RN) licensure.
- Registered Nurse Diploma.
