Role Overview

We are seeking an experienced Licensed Practical/Vocational Nurse (LPN/LVN) to join our interdisciplinary team as a Rehabilitation Nurse Specialist. As a key member of our team, you will play a vital role in delivering exceptional patient care and contributing to the success of our hospital.

Job Description

As a Rehabilitation Nurse Specialist, you will be responsible for providing high-quality patient care, conducting assessments, developing individualized care plans, and collaborating with healthcare professionals to ensure optimal outcomes. Your expertise will be invaluable in helping patients recover from various conditions, including stroke, brain injury, neurological disorders, trauma, spinal cord injuries, amputations, and orthopedic injuries.

Responsibilities:

  • Demonstrate knowledge of medications and their administration based on patient age and clinical condition.
  • Perform patient care according to established standards of care for geriatric and general inpatient rehabilitation patients.
  • Assist with patient assessments and reassessments under the supervision of the Registered Nurse (R.N.). Identify and implement appropriate rehabilitation nursing interventions.
  • Conduct timely and accurate Quality Improvement (QI) assessments under the supervision of the R.N.
  • Implement and evaluate individualized care plans based on patient needs and responses to treatment.
  • Engage with patients and families to involve them in care planning and decision-making.
  • Utilize pain management techniques and educate patients and families accordingly.
  • Perform other duties as assigned by the supervisor or leadership team.

Requirements

To be considered for this rewarding role, you must possess:

  • A diploma or degree in Nursing from an accredited Licensed Practical/Vocational Nurse program.
  • An active license to practice as a Licensed Practical/Vocational Nurse in the state where the hospital is located.
  • CPR/BCLS certification; ACLS preferred. IV Certification is highly recommended.
  • A minimum of six months' experience in Medical/Surgical settings, preferably in acute care environments.
  • Excellent oral and written communication skills, with the ability to interact effectively with patients, families, and healthcare teams.

Earn $65,000 - $75,000 per year
