Key Responsibilities
  • Patient Care Management: Conduct nursing assessments and work alongside the nursing team and other healthcare professionals to create effective care plans.
  • Goal Setting: Establish realistic and measurable short and long-term goals for patients, implementing interventions as necessary to reduce rehospitalizations and continuously evaluate outcomes.
  • Medication Administration: Administer medications and perform treatments according to physician directives.
  • Supervision: Delegate nursing responsibilities to staff and oversee their execution to ensure quality care delivery.
  • Communication: Share patient information effectively with assigned staff and during shift changes.

Qualifications

Requirements: A graduate of an accredited nursing program and currently licensed by the State Board of Nursing. CPR Certification is required. One to three years of experience in a related nursing role is preferred.
