Transplant Social Worker

The Transplant Social Worker provides comprehensive social work services to transplant candidates, recipients, living donors, and their families throughout the transplant process. This includes pre-transplant, transplant, and post-transplant phases of care. As a vital member of the transplant team, the Transplant Social Worker offers emotional support, patient education, and addresses social determinants of health. They are responsible for referrals to community agencies, collaborate and coordinate patient care with other transplant team members, and provide concrete services. Through a range of interventions and treatment modalities, the Transplant Social Worker contributes to optimal health, satisfaction, and cost-effective care by addressing behavioral, social, and environmental issues that may impede transplantation and post-transplant care.
Key Responsibilities:- Provide comprehensive assessment, counseling, interventions, planning, and referral of services to meet the psychosocial needs of patients and families.
- Round on assigned patients with the Transplant Team.
- Document all interventions and participate in patient and staff education, outreach activities, professional presentations, transplant team care conferences, and selection committee meetings.
- Participate in quality improvement and/or research projects and other relevant duties as assigned.
- MSW from a Social Work program accredited by the Council on Social Work (CSWE).
- NYS Certification (LMSW or LCSW).
- Three years of relevant post-master's experience.
- Transplant experience (preferably in a hospital and/or healthcare environment).
- Active participation in transplant-related and/or professional social work organizations.
