We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Adoration Home Health and Hospice. As a PRN (as needed) nurse, you will provide high-quality, individualized care to patients in their homes, supporting their families and promoting their overall well-being.
Responsibilities- Provide safe and effective patient care, following established plans of care and protocols.
- Deliver medications, feedings, and other treatments as needed, while maintaining patient dignity and independence.
- Monitor patient vital signs and report any abnormalities to the Case Manager and Physician.
- Document all nursing actions, communication with family and healthcare team, and patient progress.
- Coordinate and order supplies, medications, and equipment as needed.
- Ensure all patient needs are met in accordance with physician instructions.
- LPN/LPT/LVN degree from an accredited college of nursing, with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications and home health care.
- Ability to lift and carry items weighing up to 20 pounds, and to bend, stoop, push, pull, sit, and walk for periods of time.
