Job Title: RN Clinical Manager, Home Health

Join CenterWell Home Health, a leading provider of home health care services, as a RN Clinical Manager, Home Health. In this role, you will be responsible for overseeing the delivery of high-quality patient care, managing a team of clinicians, and ensuring compliance with regulatory requirements.

Key Responsibilities:
  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for a specific location.
  • Conduct assessments and reassessments of patients, including updating care plans and interpreting patient needs.
  • Manage the assignment of caregivers and ensure the delivery of care to all patients served by the location.
  • Responsible for QA/PI activities, including data tracking for performance review and outcomes of care analysis.
  • Provide direct patient care on an infrequent basis and only in times of emergency.
  • Act as Branch Director in his/her absence.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR Card.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice, or equivalent environment.
  • Home health experience, preferred (highly desired).
  • Management and people leadership experience, preferred (highly desired).
  • OASIS experience, preferred (highly desired).
  • Homecare Homebase (HCHB) experience, preferred (highly desired).
  • CMS PDGM billing knowledge and/or experience, preferred.
Benefits:
  • Competitive salary range: $76,800 - $105,800 per year.
  • Annual bonus eligible.
  • Normal hours of operation: M-F / 8a-5p (ET).
  • On-call expectation: Yes, rotating on-call shift.
  • Branch size: 1,100+ Census (3.5 STAR rating).
