The OpportunityWe're seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team in Avondale, Arizona. As a key member of our outpatient practice, you'll have the opportunity to work closely with our medical director and care team to deliver exceptional patient care and drive practice innovation. If you're passionate about geriatric care and value-based models, this is an exciting opportunity to join a dynamic team and make a meaningful impact in the lives of our patients.
Key Responsibilities- Provide comprehensive primary care to patients in an outpatient setting, leveraging our proprietary Electronic Health Record to plan care and maximize impact.
- Partner with the care team to engage our geriatric patient population in care and drive behavior change, utilizing our integrated behavioral health models and coaching services.
- Collaborate with the medical director and care team to develop and implement practice innovations that improve patient outcomes and enhance the care experience.
- Participate in weekly team-building sessions and shared discussion of challenging cases to foster a culture of collaboration and continuous learning.
- At least 2 years of primary care experience required.
- Currently licensed or ability to obtain licensure in the state of Arizona.
- Excellent communication and interpersonal skills, with the ability to build successful relationships with team members and patients.
- Ability to thrive in a fast-growing, mission-driven organization that values data-driven decision making and continuous improvement.
- Board Certification.
