The Opportunity

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in Avondale, Arizona. As a Primary Care Specialist, you will play a critical role in delivering high-quality care to our geriatric patient population.

Key Responsibilities

  • Provide comprehensive primary care in an outpatient setting, including routine check-ups, health screenings, and disease management.
  • Partner with the care team to develop and implement personalized care plans that address the unique needs of each patient.
  • Utilize our proprietary Electronic Health Record to plan care and track patient outcomes.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.

Requirements

  • At least 2 years of primary care experience required.
  • Currently licensed or ability to obtain licensure in the state of Arizona.
  • Ability to build successful relationships with team members and communicate effectively with patients and families.
  • Board Certification.
