We are seeking a compassionate and self-reliant Licensed Vocational Nurse (LVN) to join our team in the area of Home Health Care. As a Home Health Care LVN, you will apply your experience and knowledge to the needs of your clients, allowing them to remain within the familiar surroundings of their own homes.

We will provide you with support and encouragement, enabling you to be successful in this rewarding career.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Conduct thorough assessments of patients' needs and progress toward discharge goals.
  • Collaborate with healthcare professionals to establish optimal discharge plans.
  • Monitor patients' conditions and perform appropriate skilled services as indicated in the plan of care or physician orders.
  • Collect, analyze, and interpret data to document actual and/or potential nursing diagnoses.
  • Enter timely and accurate documentation into the computer.

Qualifications:

  • Current Licensed Vocational Nurse (LVN) License in the state in which the Licensed Vocational Nurse (LVN) practices.
  • Ability to travel to multiple patients per day.
  • One year prior Licensed Vocational Nurse (LVN) experience preferred.
