Position Overview:


The Ambulatory Care Pharmacist is responsible for delivering pharmaceutical care services in an outpatient setting, collaborating with healthcare professionals to manage patient medication therapies, provide education, and oversee the preparation and dispensing of medications.

This role emphasizes high-quality, evidence-based, and cost-effective pharmaceutical care, performed within designated clinical areas.

Key Responsibilities:
Adhere to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and relevant state, federal, and local regulations.

Conduct thorough medication history assessments upon referrals from healthcare providers.


Develop comprehensive treatment plans based on diagnoses, which may encompass drug therapy and lifestyle adjustments, ensuring smooth transitions across various care settings.


Identify, address, and avert potential and actual medication-related issues, including: untreated conditions, inappropriate drug choices, inadequate dosages, overdose risks, medication non-adherence, adverse drug reactions, drug interactions, and unnecessary medications.


Initiate, modify, monitor, and recommend adjustments in drug therapy under established protocols, utilizing information gathered from patient interviews, objective monitoring, and reviews of medical records.

Coordinate and interpret laboratory monitoring related to drug therapy, including serum drug levels and other approved drug-related tests.


Monitor patients to evaluate and enhance outcomes, optimize drug utilization, ensure adherence, and minimize adverse drug reactions.


Provide patient education regarding disease states, including expected effects, side effects, adherence importance, and any specific instructions.

Ensure continuous improvement in the quality of pharmaceutical care services offered.

Deliver educational sessions to providers and staff through presentations, newsletters, formal reviews, and direct interactions.

Conduct drug utilization reviews, comprehensive medication assessments, and medication reconciliations.

Utilize pharmacy computer systems and other necessary equipment and software.

Document relevant information accurately in patient medical records and other systems in compliance with policies and regulatory standards.

Foster a collaborative Labor-Management Partnership environment.

Supervise ancillary/support staff, including technicians, assistants, support coordinators, and pharmacy interns.

Perform additional duties as required.

Qualifications:
Experience


A minimum of 1200 hours of experience within the last five years working under collaborative drug therapy management protocols, including initiating, adjusting, discontinuing, and refilling medications for patients with chronic conditions, or successful completion of an ASHP Accredited PGY-1 residency.

A qualified candidate should possess at least ONE of the following credentials or experiences: Board Certified Oncology Pharmacist (BCOP), completion of a PGY2 in oncology practice, or 1000 hours of experience in sterile compounding oncology pharmacy with direct clinical protocol management, with the condition of achieving BCOP within three years of employment.

Education

A degree in Pharmacy that meets the minimum educational requirements for licensure.

License, Certification, Registration

California Pharmacist License required at hire.

Basic Life Support certification within three months of hire.

National Provider Identifier required at hire.

Additional Requirements:
Successful completion of a standardized competency assessment, with exceptions for current Ambulatory Care Pharmacists.

Knowledge Required:


In-depth knowledge of current pharmacological and biopharmaceutical principles, medical terminology, disease management, age-appropriate therapies, and relevant information pertaining to pharmaceutical care management.

Familiarity with Federal and State laws/regulations governing pharmacy services and the pharmacist's scope of practice.

Demonstrated experience in aseptic techniques and preparation of sterile IV products and hazardous agents, as necessary.

Awareness of current drug information sources and medical literature.

Skills and Abilities Required:
Proven ability to extract pertinent information from patients and other sources for effective medical management plans.

Demonstrated teaching and evaluation skills.

Effective verbal and written communication skills.

Strong analytical and problem-solving abilities.

Experience working collaboratively within a multidisciplinary patient care team.

Preferred Qualifications:
Doctorate of Pharmacy (Pharm.D.).
Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
Board certification in a relevant practice area.
Advanced Practice Pharmacist License.
