Oncology Ambulatory Care Pharmacist

Position Overview:
The Ambulatory Care Pharmacist is responsible for delivering comprehensive pharmaceutical care services in collaboration with healthcare professionals. This role involves managing patient medication therapies, providing education to patients, and overseeing the preparation and dispensing of medications.
Key Responsibilities:
Adheres to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable laws.
Conducts thorough medication history assessments upon referrals from healthcare providers.
Develops treatment plans based on diagnoses, which may encompass drug therapy and lifestyle changes, ensuring continuity of care.
Identifies, addresses, and prevents medication-related issues, including untreated conditions, inappropriate drug selections, and adverse drug reactions.
Initiates, adjusts, and monitors drug therapies according to established protocols, utilizing patient interviews and medical records.
Monitors patients to evaluate and enhance outcomes, ensuring compliance and minimizing adverse drug reactions.
Educates patients on their conditions, including expected effects, side effects, and the importance of adherence to treatment.
Delivers educational sessions to healthcare providers and staff through various formats.
Conducts drug utilization reviews and medication reconciliations.
Operates pharmacy information systems and other necessary equipment.
Documents relevant information in patient medical records and other systems in accordance with policies and regulations.
Supports a collaborative work environment.
Supervises ancillary staff, including technicians and pharmacy interns.
Performs additional duties as needed.
Qualifications:
Experience
A minimum of 1200 hours of experience in the last five years working under collaborative drug therapy management protocols, or successful completion of an ASHP Accredited PGY-1 residency.
Education
A degree in Pharmacy that meets the minimum educational requirements for licensure.
License, Certification, Registration
California Pharmacist License required upon hire.
Basic Life Support certification within three months of hire.
National Provider Identifier required upon hire.
Additional Requirements:
Successful completion of a standardized competency assessment, with exceptions for current Ambulatory Care Pharmacists.
Knowledge and Skills Required:
In-depth knowledge of pharmacological principles, medical terminology, disease management, and therapy appropriate for various age groups.
Demonstrated experience in aseptic techniques and preparation of sterile IV products as required.
Familiarity with current drug information sources and medical literature.
Skills and Abilities:
Ability to extract relevant information for medical management plans.
Proficient in teaching and evaluating others.
Strong verbal and written communication skills.
Demonstrated analytical and problem-solving abilities.
Experience collaborating within a multidisciplinary patient care team.
Preferred Qualifications:
Doctorate of Pharmacy (Pharm.D.).
Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
Board certification in a relevant practice area.
Advanced Practice Pharmacist License.
