Overview
Rush is a leading healthcare provider seeking a skilled Clinical Nurse Navigator RN to join our team. This role will play a critical part in improving patient outcomes through care coordination, follow-up, and support.

About the Role
This position involves providing consultation in specialty areas to healthcare providers, guiding patients on treatment plans, and arranging appointments. The successful candidate will work with participating sites to share patient data, complete electronic records, and develop relationships with community leaders.

Responsibilities
• Provides consultation in specialty areas to healthcare providers
• Offers guidance and emotional support to patients regarding decisions on follow-up and treatment plans
• Arranges appointments with patients, reviews test preparation procedures, and assesses potential patient barriers
• Contacts patients via phone calls, emails, and letters to remind them about appointments, assists in scheduling/rescheduling appointments, and answers questions about testing and treatment
• Performs follow-up phone calls to patients after appointments as indicated
• Works with participating sites to develop mechanisms for sharing patient data in a manner that utilizes their existing data and optimizes their time and effort

Requirements
The ideal candidate will possess a current State of Illinois Registered Nurse licensure, Bachelor's degree in Nursing or Master's degree in Nursing, and at least three years of registered nurse experience. Excellent written and verbal communication skills, high level of interpersonal skills, and proficiency with Microsoft Office Suite are essential. Ability to adapt to change, work independently, and integrate as part of a team are also required. Additionally, cultural competency, resourcefulness, resilience, and innovation problem-solving skills are desirable.

Compensation and Benefits
We offer an estimated salary range of $80,000 - $110,000 per annum, commensurate with experience.
