Position Overview:
Oversee and guide the case management social work team. Responsible for the initial assessment, triage, and allocation of patients to case managers, ensuring the coordination of all necessary systems and services for a well-organized, multidisciplinary, patient-centered care approach. Ensure the delivery of quality, cost-effective care for the designated patient demographic. Provide leadership and mentorship to the department staff, serving as a resource and role model. Act as a liaison for patients, families, healthcare team members, community resources, and staff. Ensure compliance with hospital and departmental policies and procedures. Patient care assignments may encompass Neonate, Pediatric, Adolescent, Adult, and Geriatric age groups.

Key Responsibilities:
* Patient-Centered Care: Uphold and advocate for the core principles of the Patient-Centered Medical Home or Patient-Centered Specialty Practice as applicable.
* Leadership: Lead by identifying challenges and opportunities for enhancement, program development, implementation, and assessment.
* Workload Management: Supervise and allocate case loads and staff hours to meet the hospital's needs effectively.
* Performance Standards: Assist in formulating performance standards, evaluating staff performance, and recommending personnel actions as necessary.
* Care Planning: Create comprehensive multidisciplinary care plans utilizing available tools and resources effectively.
* Data Management: Conduct assessments, gather data, and analyze information in collaboration with patients, families, and healthcare team members.
* Clinical Assessment: Evaluate the clinical and psychosocial status of patients and their current treatment plans.
* Needs Assessment: Identify the needs of patients and families concerning medical diagnoses and treatment options, including financial and psychosocial resources.
* Collaboration: Foster collaborative relationships with other departments and community health agencies to support quality care in clinical expertise.
* Discharge Planning: Facilitate timely discharge planning by anticipating patient needs in collaboration with healthcare providers.
* Intervention Monitoring: Track and assess patient responses to interventions in partnership with quality assurance and utilization review.
* Variance Management: Analyze patterns of variance from established care protocols and implement resolution strategies.
* Patient Education: Provide instruction to patients and families based on identified learning needs, assessing their knowledge and health expectations.
* Information Dissemination: Assist in developing methods to ensure effective communication among healthcare team members.
* Continuity of Care: Collaborate with the healthcare team to maintain continuity of patient care across all settings.
* Supervisory Duties: Establish an efficient organizational structure, supervise staff, and manage hiring, training, evaluations, and disciplinary actions.

Primarily sedentary work, requiring occasional lifting of up to 10 pounds and the ability to sit for extended periods, with brief walking or standing as needed.

Working Conditions:
Minimal physical hazards in an office environment; may require travel to various work sites and participation in on-call duties, including rotating shifts, holidays, and weekends.
