Clinical Nurse Coordinator

We are seeking a skilled Clinical Coordinator to join our team at DaVita. As a Clinical Coordinator, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease or chronic kidney disease.
Key Responsibilities:- Coordinate patient care and services, including tracking vital signs, lab values, and outcome management.
- Ensure the safety, comfort, and well-being of patients by delivering high-quality care.
- Collaborate with the clinical team to achieve quality improvement initiatives and clinical goals.
- Provide training and support to staff RNs and PCTs.
- Oversee patient care and services, including patient observations, measuring stats, and machine setup.
- Current Registered Nurse (RN) license in the state of practice.
- At least 18 months of registered nursing experience.
- Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
- Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
