Responsibilities:
  • Provide high-quality, patient-centered care in a home health setting, utilizing the nursing process to coordinate and deliver care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans, ensuring seamless transitions and optimal outcomes.
  • Conduct thorough patient assessments, identifying physical, psychosocial, educational, and safety needs, and develop targeted interventions to address these needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure continuity of care and promote patient advocacy.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that enhance patient care and outcomes.
Requirements:
  • Graduation from a school of professional nursing.
  • Current licensure as a Registered Nurse in the state of North Carolina.
  • One year of nursing experience in an acute or chronic care setting, or an equivalent combination of education and experience.
