About the Position

  • Work under the supervision of the Ambulatory Pharmacist Program Medical Director to provide population health-focused pharmacy services
  • Collaborate with central and local level population health teams to develop and implement strategies for high-value, high-quality patient care
  • Develop and implement patient education initiatives related to disease states and medication adherence

Responsibilities

  • Conduct patient medication history reviews and offer alternative therapeutic suggestions or changes to regimen
  • Work with population health teams and providers to improve performance on value-based payer contracts

Requirements

  • PharmD from an ACPE-accredited College of Pharmacy required
  • Current PA Registered Pharmacist License; Basic Life Support (BLS) card preferred
  • 1+ years of patient care delivery experience required if a clinical pharmacy residency was not successfully completed, 3+ years preferred

Compensation

The estimated salary for this position is $120,000 - $160,000 per year, based on industry standards and location.
