About the Role

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team. As a key member of our care coordination team, you will be responsible for providing exceptional patient care and support to individuals in the geriatric population.

Key Responsibilities
  • Serve as the primary point of contact for patients, families, and healthcare providers to coordinate and facilitate care plans.
  • Conduct thorough assessments of patients' medical, social, and emotional needs to identify opportunities for improvement and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care and timely interventions.
  • Maintain accurate and up-to-date records of patient interactions, including progress notes, phone calls, and emails.
  • Develop and implement strategies to improve health literacy, patient engagement, and self-management skills among patients and their caregivers.
  • Prioritize and manage high volumes of patient communication, ensuring prompt responses to urgent matters and resolving complex issues in a timely manner.
  • Utilize clinical expertise to provide education and guidance on disease management, medication adherence, and healthy lifestyle choices.
Requirements
  • Bachelor's degree in Nursing (BSN) or Associate Degree in Nursing (ADN) with current RN licensure in all states where Cigna operates.
  • A minimum of 3 years of experience as a Registered Nurse, preferably in gerontological nursing or a related field.
  • Proven ability to work independently and collaboratively as part of a multidisciplinary team.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Proficiency in electronic medical records (EMRs), Microsoft Office applications, and other relevant software systems.
Preferred Qualifications
  • Gerontological nursing certification (e.g., GNP-BC or AGNP-C).
  • Experience working with home health agencies, hospice care, or palliative care services.
  • Strong knowledge of healthcare regulations, policies, and procedures.
  • Ability to speak multiple languages, particularly those commonly spoken by the aging population.
  • Knowledge of quality metrics and performance improvement initiatives.
Salary Information

The estimated annual salary for this position is $75,000-$90,000, depending on experience and qualifications.
