City Pro Group, Inc. is a leading provider of early intervention services to children with developmental delays and disabilities. We are seeking a highly skilled and compassionate Service Coordinator to join our team in our Bronx office.



Responsibilities:

  • Guide families through the early intervention process, ensuring they receive the necessary support and resources.
  • Provide exceptional case management services to young children with special needs and their families in community and office-based settings.
  • Support families through the evaluation process, advocating for their child's needs and securing authorized services.
  • Monitor the quality of services provided, ensuring the highest standards are met.
  • Attend home visits and meetings as scheduled, maintaining open communication with families and stakeholders.
  • Help families navigate the transition between services, ensuring a seamless experience.
  • Ensure the implementation of all authorized services, eliminating gaps in care.
  • Prepare and submit daily session notes and other paperwork in a timely and accurate manner.
  • Provide services in accordance with Early Intervention Regulations, upholding the highest standards of care.


Qualifications:

  • Prior experience in Service Coordination is required.
  • Bachelor's degree in Social Services Education or a related field (e.g., Psychology, Social Work, Education, Sociology).
  • Bilingual Spanish is preferred.
  • Able to commute within all five NYC boroughs (Manhattan, Bronx, Brooklyn, Queens) via car or public transportation.
