Position Overview:
This role is situated within our Case Management team and focuses on enhancing the Progression of Care, Discharge Planning, and Interdisciplinary Rounding to ensure safe transitions for patients to subsequent levels of care. Weekend and holiday rotations may be required on an as-needed basis.- The primary objective is to support physicians and interdisciplinary teams in facilitating optimal patient care, thereby improving clinical outcomes and patient satisfaction while ensuring effective and timely utilization of inpatient services throughout the transition of care.
Key Responsibilities:
- Assist patients and their families in navigating the healthcare system, including post-acute options and insurance plans, to coordinate necessary services.
- Collaborate with the interdisciplinary team to continuously assess the patient's evolving needs.
- Organize and lead care conferences involving patients, families, representatives, and other care team members. Participate in ethics committee meetings as required.
- Engage in discharge planning, which includes coordinating patient transfers to other facilities and connecting patients with community resources. Provide discharge education and resource referrals, addressing any questions or concerns regarding transitions of care.
- Work closely with governmental agencies (such as DCF, APS, CMS, and KePro) to ensure safe post-acute care and adherence to regulatory standards.

- ACCOUNTABILITY, ANALYTICAL THINKING, CLINICAL POLICIES AND STANDARDS, CUSTOMER SERVICE, DISCHARGE PLANNING, CLINICAL EFFECTIVENESS, PATIENT AND FAMILY CENTERED CARE, ADAPTABILITY, PROFESSIONAL STANDARDS, TEAM COLLABORATION.Education and Certification Requirements:
- Graduate of an accredited Registered/Professional Nursing Program (Required).- Basic Life Support (BLS) certification from the American Heart Association (Required).
- Valid Registered Nurse License (RN LICENSE) in the State of Florida (Required).
Additional Information:
- Complexity of Work: This position necessitates critical thinking, effective communication, decisive judgment, and the ability to cultivate positive relationships. The incumbent must demonstrate leadership skills and take appropriate actions when necessary. Education and resources relevant to the effective progression of care, service utilization, appropriate care levels, and safe patient transitions must be provided to patients, families, and the healthcare team.
- Required Experience: A minimum of one (1) year of experience in hospital-based Case Management practice or five (5) years in the healthcare sector is preferred.
Working Conditions:
- Primarily an office-based role with no eligibility for shift differentials.
