Job Summary

The Medical Social Worker LCSW provides clinical counseling and consulting services to patients, families, and staff members, and assists in facilitating the development of new and expanded social work services and programs within the hospital and plans for discharge and aftercare.

Key Responsibilities
  • Interview patients and family members to obtain relevant bio-psychosocial information and determine the effect of illness and trauma upon patient and patient's family.
  • Develop and implement a social work treatment plan for patients and family members based on an analysis of patient's psychosocial factors and medical illness.
  • Utilize appropriate treatment modalities, including individual, family, and group intervention, to provide counseling to patients and families in areas such as family problems, coping with illness, trauma, loss of function, and grief.
  • Participate as part of the multidisciplinary team in case and family conferences to assist in formulating overall patient care plans and interpret psychosocial aspects of the patient's illness to other staff.
  • Provide community and professional staff training regarding patient psychosocial needs and/or coping with illness, grief, and loss.
  • Provide evaluation, support, and follow-up referrals, planning, and reports as needed by all victims of child abuse, rape, elder abuse, and family violence.
  • Provide evaluation and referral services for patients who are alcohol or drug dependent.
