About the Role:
We are seeking a highly skilled and compassionate Hospital Case Manager Social Worker to join our team at Clinical Management Consultants. As a key member of our healthcare organization, you will play a vital role in supporting our patients and their families through the utilization review process.
Key Responsibilities:
- Collaborate with multidisciplinary teams to evaluate hospital resource use and identify areas for improvement.
- Work closely with UR and CM specialists from the Health Plan to develop and implement shared resources and workflow processes.
- Provide patient navigation, education, and community resource support to patients and their families.
- Interface with external stakeholders to ensure seamless care coordination and optimal patient outcomes.
