We are hiring a skilled and compassionate Registered Nurse for our Homecare team in Sheboygan. The successful candidate will have the opportunity to work with patients in their homes, providing high-quality care and building meaningful relationships.

The Registered Nurse is responsible for coordinating patient care plans, ensuring continuity of care and collaborating with other healthcare professionals. This role requires a strong clinical background, excellent communication skills and a passion for delivering exceptional patient outcomes.

Key responsibilities include:

  • Providing nursing services to patients in their homes, as prescribed by physicians or authorized by state/program regulations
  • Developing and revising individualized care plans, taking into account federal and state program requirements
  • Ensuring patient eligibility and medical necessity for services, meeting agency policy and payer source requirements

To succeed in this role, you will need:

  • A minimum of one year experience as an RN, preferably in home healthcare or a related field
  • Current RN licensure in the state of practice, along with CPR certification
  • A reliable vehicle, driver's license and vehicle insurance, or access to public transportation

This position offers a competitive salary range of $65,000 - $85,000 per annum, depending on experience, plus benefits including comprehensive health insurance, paid time off and professional development opportunities.
