Job Title: LPN Licensed Practical Nurse

We are seeking a skilled LPN Licensed Practical Nurse to join our team at Belmont Village Glenview. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Job Summary:

The LPN Licensed Practical Nurse will be responsible for administering medications, supporting wellness initiatives, and ensuring regulatory compliance. You will communicate effectively with healthcare practitioners, ancillary providers, pharmacies, family members, and staff to ensure timely clinical interventions and follow-up.

Responsibilities:
  • Follow up on physicians' orders, coordinate services with ancillary providers, and report to physicians and families any changes of condition or other pertinent clinical care information.
  • Provide medication administration to an assigned group of residents according to physicians' orders and report any adverse reactions or clinical observations, documenting as necessary.
  • Ensure resident care and service plans are followed by overseeing tasks of Medication Technicians and care providers.
  • Make appropriate situational decisions and manage conflict in a positive and constructive manner.
  • Update and review resident medical records, clinical documentation, and incident reports.
Requirements:
  • Active LVN or LPN license in the State in which this community is located.
  • CPR Certified or ability and willingness to obtain within 30 days of hire.
  • Proficient in verbal and written English with strong oral and written communication skills.
  • Ability to work the defined schedule for this position, which may include weekends and holidays.
  • Intermediate computer and mobile device skills.

  • Dependable and self-directed with strong organizational and time management skills.
  • Strong leadership and supervisory abilities, adept at guiding and directing teams.
  • Works with a sense of urgency while paying close attention to detail.
  • More than one year of medication management experience.
  • Direct supervisory experience.
